| For group class we require that your dog be vaccinated against Kennel Cough (Bordetella) and Rabies (for dogs over 6 months old), Parvo, Distemper, Corona, etc. (usually called 5 in 1, example DHLP). Puppies usually require a series of these vaccines between 2 months and 5 months... the vaccine must be given every two to three weeks during this period as directed by your veterinarian. It is crucial that you have this done for your puppy's safety to protect him/her from contagious and possible fatal diseases transmitted, or carried, by other dogs or their owners. To help things go faster the first day of class, please mail a copy of your dog's current vaccination record with this form. | |||||||

| You will not be called unless we cannot accommodate you. If you need further information or to confirm receipt of your check please call us at (818) 996-3647. You will need a six foot leash, your dog's favorite treats, a collar your dog cannot slip out of, a towel for them to lay on, a folding chair if you don't want to stand and complete copies of shot records for your dog at first class. Fun nose work students need to bring a crate. Please do not bring sick or aggressive dogs to dog training class. If you do not know if your dog will be a problem around others, contact us for a free evaluation. If your dog is sick or injured you may attend class without your dog until he/she is well again! If class is cancelled or changed due to low enrollment your class fees can be refunded or applied to another session of your choice! If you are repeating a class please fill out the form and mail, email or fax it to us to confirm your attendance in the class you would like. 72 hour notice is required to cancel with a refund or to change to another class location once you have pre-registered. During the course of class you will not be allowed to change location or level without repayment. Remember repeat spaces are limited, if you are holding a space for repeat , you must attend the class for which you have registered or you will need to pay 1/2 price to reinstate. No one else can have your spot once it has been given away. Be courteous to others and let us know if you will not be able to attend the full course. | ||||||
